Pyrazinamide 20–40   (2 g/d) 50–70   (4 g/dose)
Ethambutol 15–25   (2.5 g/d) 50   (2.5 g/dose)

*If isoniazid plus rifampin are to be given together on a daily basis, the doses should be 10 mg isoniazid per kilogram and 15 mg rifampin per kilogram.
