Integrity declaration form

Complete this form only when you meet all three conditions:

1. You are a government supplier
2. You, one of your affiliates or a proposed first-tier subcontractor has been charged with or convicted of a criminal offence in a country other than Canada and to the best of your knowledge and belief, the offence may be similar to one of the listed offences in the Ineligibility and Suspension Policy
3. You are unable to provide any of the certifications required by the integrity provisions
