I, being the author and/or owner of copyright in the Work, and wishing to have the Work published and distributed by Biological Procedures Online/4012038 Canada Inc., including but not limited to the journal Biological Procedures Online and its archives, in any medium, ("the Publication"), in consideration of the exchange of good and valuable consideration, the receipt and sufficiency of which is irrevocably acknowledged, make the following representations and warranties:

1. I have made all necessary inquiries, and have obtained all necessary copyright licenses and authorizations to include material in the Work for which other persons own copyright.
2. I have made all necessary inquiries, and have identified to Biological Procedures Online all authors and owners of copyright in the Work.
3. I have made all necessary inquiries, and the list of authors fairly conforms to academic conventions regarding who should be included as an author.
4. I have informed all copyright owners in the Work of this Copyright License, and I have obtained authorization from all copyright owners to execute this agreement on behalf of all copyright owners in the Work.
5. I will indemnify Biological Procedures Online from any and all claims arising from publication of the Work, including but not limited to libel and copyright or moral rights infringement.
6. I represent and warrant that the Work has not been published previously.

I hereby grant a non-exclusive license to Biological Procedures Online to publish the Work in the Publication. This license shall be irrevocable upon publication of the Work.

I understand that submission of this form does not guarantee publication. In the event that the Work is withdrawn by the applicant(s) before publication, or is not accepted for publication, this agreement becomes null and void.

This Agreement shall be governed by and construed in accordance with the laws of the Province of Ontario, Canada.
