Call for Nominations of Canadian Best Practices in Health Promotion and Chronic Disease Prevention

The focus for Phase two:

The initial search for content will focus on health promotion and chronic disease prevention interventions, including topics such as:

  • mental health (in relation to settings);
  • obesity (in relation to protective and risk factors of activity, nutrition, tobacco, alcohol);
  • newcomers/immigrant health (as related to general chronic disease); and
  • chronic diseases, with a primary or secondary prevention focus (particular interests in cancer, cardiovascular disease and diabetes). Chronic disease and health promotion are considered underlying main themes and therefore apply to all aspects of content searching and updating.

All topics must consider at least one or more priority determinants of health.

In this second phase, the portal’s collection of interventions will be expanded to include not only "best" practices, but also "promising practices", including innovative programs with varying levels of evidence and that have shown success. They are actively seeking to add Canadian interventions onto the Portal as well.

In the spirit of knowledge exchange, the Portal is looking to the healthy living community to nominate effective interventions, practices and programs, resources and/or partners/links to be added onto the Portal. Please also contact Nina to obtain updated PDFs to the selection process (Methodology), where you will find information regarding the criteria used to screen nominations. All content submitted will be vetted by a group of experts against the selection criteria. Users who nominate interventions, practice and program information will receive an automated response confirming receipt of their nomination.
