Canada's new Adverse Event Following Immunization Report Form

On December 1, 2008, the new Adverse Event Following Immunization Report (AEFI) Form will be posted on the Public Health Agency of Canada's Vaccine Safety web site at: http://www.phac-aspc.gc.ca/im/aefi-form-eng.php

Please note that the form will also be published in the 2008 Compendium of Pharmaceuticals and Specialties (CPS).

Similarly, hardcopy of the form will be distributed to provincial and territorial jurisdictions.

Until that time, the current form should be used for reporting adverse events following immunization.
