By completing the School Demographics Questionnaire, you will be establishing a point of reference before administering the Student Needs Assessment Questionnaire. This assessment should be completed by the project leader, or key contact, and will:

• Be a picture of the school community as it stands before the implementation of any programs;
• Provide useful information when presenting the final results from the Student Needs Assessment Questionnaire; and
• Enable you to gather background information on the school that can be used to compare future findings.

Much of the information asked on the School Demographics Questionnaire will be available from a school profile that may already exist on the school web page, or have been prepared by the administration and the parent council, or available from the office/school principal.
