- You need the signed consent of the person you are enquiring about, or, if the person is deceased, you need the signed consent of the administrator or executor of the person's estate unless you are applying on behalf of: a) an organization that has an information-sharing agreement with the Department of Justice to obtain information in the Central Registry of Divorce Proceedings (CRDP) databank; or b) an investigative body listed in Annex II of the Privacy Regulations, SOR/83-508. (There is a space at the end of this form for the person to sign to provide consent.)

CONSENT FOR USE AND DISCLOSURE OF PERSONAL INFORMATION
(to be completed and signed by the person whose information is being requested or by the executor or administrator of the person's estate if the person is deceased and another person is making the request on the executor's or administrator's behalf).
The CRDP databank (JUS PPU 005) collects and stores information under the authority of the Central Registry of Divorce Proceedings Regulations, SOR/86-600. The purpose of this information is to detect duplicate pending divorce applications so that the courts where the applications were filed can settle jurisdictional issues under section 3 of the Divorce Act, R.S.C.. 1985, c.-3 (2nd Supp.). This information is protected, used and disclosed in accordance with the Privacy Act,.R.S.C., 1985, c. P-21.
The CRDP has been asked to disclose personal information about you, or about a deceased person whose estate you are administering, to the person, institution or organization indicated above. Under the Privacy Act, the CRDP cannot disclose this information without your written consent. It is your choice whether or not to provide consent. You will not be penalized if you choose not to sign the form.
