Import Inspection Report (Canadian Food Inspection Agency/Agence canadienne d'inspection des aliments 1422)

This form, Import Inspection Report (Meat and Poultry Products), is provided by the Government of Canada through the Canadian Food Inspection Agency.

The information you provide on this document is collected by the Canadian Food Inspection Agency under the authority of the Meat Inspection Act or the Canada Agricultural Products Act for the purpose of food safety. Some information may be accessible or protected as required under the provisions of the Access to Information Act. Information that could cause you or your organization injury if released is protected from disclosure as defined in section 20 of the Access to Information Act.

Section 1 - Port has nine fields to fill out:

  1. Certificate number
  2. Country of origin, which is indicated by the user by a three-letter code
  3. Port of entry
  4. Product description (if applicable)
  5. Transport company
  6. Destination
  7. Inspector's name
  8. Signature
  9. Date (indicated by the user in the format year, month, day)

Section 2 - Inspection has ten fields to fill out:

  1. Inspection at: (establishment number, name, location)
  2. RESULT OF INSPECTION: A) Inspection accepted totally
  3. RESULT OF INSPECTION: B) Shipment refused totally OR partially (check off appropriate box)
  4. Refused weight (kilograms) or units (boxes)
  5. Report of inspection attached (yes or no)
  6. Description of refused product (name and quantity)
  7. Reason(s) for refusal (including the section of Act and/or Regulations)
  8. Inspector's Name
  9. Signature
  10. Date (indicated by the user in the format year, month, day)

Section 3 - Notice to remove product from Canada has nine fields to fill out:

  1. In accordance with Section 18 of the Meat Inspection Act or Section 30 of the Canada Agricultural Products Act you, (name)
  2. the importer of the product described in Section 2 above and held at: (place)
  3. are hereby notified that the said product has been imported into CANADA in contravention of the Meat Inspection Act and Regulations or the Canada Agricultural Products Act and Regulations, and in particular as stated above in the "Reason for Refusal," and you are required to remove the said products from Canada within the period of ninety (90) days from the date of delivery of this notice. If the said product is not removed from Canada within the said 90-day period, it shall be forfeited to Her Majesty in Right of Canada and may be disposed of as the Minister of Agriculture and Agri-Food may direct at your expense.
