Grower/Company No. - Individual Company/Grower number assigned by the C.S.G.A.

C.S.G.A. Sequence No. - Number assigned by the C.S.G.A. used to identify and track all crop inspection reports.

Field No. - A number assigned by the grower to identify a piece of land.

Crop Code - A code assigned by C.S.G.A. identifying the variety and kind of crop.

Acres - The size of the grower's field measured in acres.

Grower/Company Name - The complete name of the Grower/Company that is requesting inspection.

Variety Name - The name of the variety that is being inspected.

  1. Indicate whether there were inaccuracies in the maps and/or information sheets provided by the applicant. If yes, correct the map and information.
  2. Indicate whether volunteer corn plants were removed before commencement of the pollination period. If not, give dates and percentages.
  3. Indicate if all off-types and rogues were removed from inbred parent rows prior to pollination. If no, give details below.
  4. Indicate if the field is affected by contamination of pollen from adjacent fields or gardens by not meeting the isolation requirements set out in Circular 6. If so, give details below.
  5. Indicate whether the field is affected by faulty performance. If so, give details below.
  6. Indicate whether, with reference to fields 4 and 5 above, if the field is affected throughout or localized. Indicate details on a map and below.

Inspection Date - Note the date of any inspections for volunteer corn on corn.

Acreage - Note the acreage observed for each inspection of volunteer corn on corn.

Adjacent source of contamination - Indicate any adjacent sources of contamination observed during the indicated inspection.
