United States Shellfish Dealer's Shipping Tag

• dealer name
• certificate number
• dealer address
• original shipper's certificate number (if different than above)
• harvest date
• harvest location
• type of shellfish, and
• quantity of shellfish.

Statement at bottom : This tag is required to be attached until container is empty and thereafter kept on file for 90 days.

Statement on side : Retailers, inform your customers: Thoroughly cooking foods of animal origin such as beef, eggs, fish, lamb, poultry, or shellfish reduces the risk of foodborne illness.
