United States Department of Agriculture/Animal and Plant Health Inspection Service Health and Export Certificate - inedible non-ruminant byproducts (intended for animal feed)

This certificate is for Veterinary purposes only. It is valid for 30 days after the date of signature (in the case of transport by ship or rail, the time is prolonged by the time of the voyage). This is to certify that rinderpest, foot-and-mouth disease, classical swine fever, swine vesicular disease, African swine fever, and contagious bovine pleuropneumonia do not exist In the United States of America.

The form gives an additional declaration:

This office has on file a notarized affidavit from [company name] verifying the accuracy of the statements below:

The inedible non-ruminant byproducts certified hereon:

  1. were derived exclusively from animals which received ante-mortem and post-mortem inspection in a federally inspected establishment;
  2. did not show evidence of pathological condition at the time of collection;
  3. were hygienically prepared and handled in accordance with United States laws and regulations;
  4. are intended for (animal) pet food use only; and
  5. were determined to be suitable for inclusion in (animal) pet food in accordance with United States Department of Agriculture regulations.

The form shows a stamp of the United States Department of Agriculture and has a number of fields that require filling in, such as signature of endorsing official, type name, title of endorsing official, description of the consignment, name and address of the exporter, name and address of the consignee, product, identification, et cetera.

The product field says: inedible non-ruminant byproduct (intended for animal feed) **include name/establishment # of Food Safety and Inspection Service facilities product is obtained from.
