United States Department of Agriculture/Animal and Plant Health Inspection Service Health and Export Certificate - inedible bovine meat/meat byproducts (intended for animal feed)

The fifth page of the form has an additional declaration:

This office has on file a notarized affidavit from [company name] verifying the accuracy of the statements below:

The inedible bovine meat/meat products certified hereon:

  1. were derived exclusively from animals which received ante-mortem and post-mortem inspection in a federally inspected establishment;
  2. did not show evidence of pathological condition at the time of collection;
  3. were hygienically prepared and handled in accordance with United States laws and regulations;
  4. are intended for pet food use only;
  5. were determined to be suitable for inclusion in (animal) pet food in accordance with United States Department of Agriculture regulations;
  6. originate from animals which were verified to be less than 30 months of age in accordance with prescribed verification methods;
  7. were produced in establishments that are certified to Canadian Food Inspection Agency as eligible for export to Canada; and
  8. do not contain any small intestines or tonsils.

The fifth page of the form shows a stamp of the United States Department of Agriculture and has a number of fields that require filling in.

The product field says: inedible bovine meat/meat byproducts (intended for animal feed) **Include name/establishment number of Food Safety and Inspection Service facilities product is obtained from.
