The overall performance of the approved facility is rated by the inspection team following each inspection conducted. Appendix 9 shows a Flow Chart table that resumes the rating used to determine the future inspection frequencies. The flow Chart is an overview of the information provided in the section 2.1.5 CFIA Inspection, 2.3 Non-Compliance, Appendix 5: CFIA Inspection - Requirements for the Inspection of Approved Facilities and Appendix 6: Classification of Non-conformance. If you select "Application" under the "Status of Greenhouse" column, then your "Frequency of Inspection Per Year" is "Quality Evaluation and Documentation Review". If your "Type of Non-conformance" is "Critical, major or minor", your "Resulting Inspection Status" is consequently "Uncertified, facility must resubmit Pest Management Plan or change records retention processes". Alternatively, if your "Type of Non-conformance" is "Nil", your "Resulting Inspection Status" would be "Approved Status".
If you select "Approved Status (A)" under the "Status of Greenhouse" column, then your "Frequency of Inspection Per Year" is "1 inspection per month for the first three months of production during the first calendar year of approbation". If your "Type of Non-conformance" is "Critical or more that 2 major", your "Resulting Inspection Status" is consequently "Suspension. Facility must complete all corrective actions ordered". Alternatively, if your "Type of Non-conformance" is "Less than 2 major or more than 4 minor", your "Resulting Inspection Status" would be "Modified Status". Although, if your "Type of Non-conformance" is "Less than 4 minor non-conformities" your "Resulting Inspection Status" is "Approved Status (B)".
If you select "Approved Status (B)" under the "Status of Greenhouse" column, then your "Frequency of Inspection Per Year" is "1 surveillance inspection every six weeks of production during the remaining nine months of the first calendar year of designation. One system inspection at the completion of the first year of designation". If your "Type of Non-conformance" is "Critical or more than 2 major", your "Resulting Inspection Status" is consequently "Suspension. Facility must complete all corrective actions ordered". Alternatively, if your "Type of Non-conformance" is "Less than 2 major or more than 4 minor", your "Resulting Inspection Status" would be "Modified Status". Although, if you select "Less than 4 minor non-conformities" as your "Type of Non-conformance", your "Resulting Inspection Status" is "Approved Status (B) during the first year of designation and until all non-conformities are corrected".
If you select "Standard Status" under the "Status of Greenhouse" column, then your "Frequency of Inspection Per Year" is "1 surveillance inspection per 3 months of designation and 1 system inspection per annum". If your "Type of Non-conformance" is "Critical or more than 2 major", your "Resulting Inspection Status" is consequently "Suspension. Facility must complete all corrective actions ordered". Alternatively, if your "Type of Non-conformance" is "Less than 2 major or more than 4 minor", your "Resulting Inspection Status" would be "Modified Status". Although, if you select "Less than 4 minor non-conformities" as your "Type of Non-conformance", your "Resulting Inspection Status" is "Standard or Modified Status (determined in consultation with the approved program officer)".
If you select "Modified Status" under the "Status of Greenhouse" column, then your "Frequency of Inspection Per Year" is "Any number of inspections to verify that corrective actions have been implemented, changes to pest management plan or to the records and documentation processes have rectified the non-conformities". If your "Type of Non-conformance" is "Critical or more than 2 major", your "Resulting Inspection Status" is consequently "Suspension. Facility must complete all corrective actions ordered". Alternatively, if your "Type of Non-conformance" is "Less than 2 major or more than 4 minor", your "Resulting Inspection Status" would be "Continued Modified Status". Although, if you select "Less than 4 minor non-conformities" as your "Type of Non-conformance", your "Resulting Inspection Status" is "Approved Status (A)".
