CPNP Site Profiles

CPNP sites are as diverse as the communities they serve.  However, all CPNP sites are united by the program’s common objectives, essential program elements and guiding principles that unify the approach to program delivery.

The following profiles demonstrate the innovation, accomplishment and success of community-based CPNP sites. The views expressed in these stories are those of the authors and individuals working in CPNP sites, and do not necessarily reflect the views of the Public Health Agency of Canada.
