Section 5 - Exchanging certificates

5.1 This section is about

exchanging replacement certificates when previously issued certificates or pink transmission copies have not been accounted for.

5.2 Use Form from CPC-Sydney

A form is sent by CPC-Sydney when a replacement certificate is to be exchanged at the citizenship office or where all previously issued certificates/pink transmission copies have not been accounted for.

On receiving the form, citizenship officers—before releasing a new certificate—must:

  • contact the applicant
  • obtain previously issued certificate(s)/pink transmission copy or indicate on the CPC-Sydney form what happened to the previously issued certificate(s)/
  • both applicant and the officer must sign this note
  • if applicable, tell the applicant that if the previously-issued certificate is found, it must be returned to the citizenship office to be sent to CPC-Sydney
  • send the completed form (with previously issued certificate(s)/pink transmission copy, if obtained) to CPC-Sydney.

Note: the applicant must answer Question 9 on the proof application as completely as possible. Entries such as "lost at home" or "misplaced" are not acceptable. For example, if a certificate is "lost at home," the application must clearly indicate that a thorough search was made to find the certificate.
