Cash purchase ticket form 6. Canada Grain Regulations, Schedule V., sections 44 and 45
44. A grain receipt or a cash purchase ticket that is required by subsection 78(2) of the Act to be issued by the operator of a licensed process elevator on the receipt of grain from a producer into the elevator shall be issued on delivery of the grain by the producer and shall be in Grain Receipt or Cash purchase ticket of Schedule V, as appropriate.
45. A grain receipt or a cash purchase ticket that is required by subsection 81(1) of the Act to be issued by a licensed grain dealer with respect to the purchase of western grain from the producer of the grain shall be issued at the request of the producer and shall be in Grain Receipt or Cash purchase ticket of Schedule V, as appropriate.
Combined primary elevator receipt form 7. Canada Grain Regulations, Schedule V., subsections 33(1) and 39(1).
33. (1) Every cash purchase ticket or primary elevator receipt issued by the operator of a licensed primary elevator shall be in the appropriate form set out in Schedule V.
39. (1) If, in accordance with section 63 of the Act, grain is lawfully offered for storage at a licensed primary elevator and a request is made that it be cleaned, the elevator receipt that is required to be issued under that section shall be a form 2 in Special bin primary elevator receipt of Schedule V, or a form 7 in Combined primary elevator receipt of that Schedule, endorsed as follows: "To be cleaned before being shipped or settled for and no shrinkage to be deducted until the surrender of this receipt."
When a licensee wants to make a direct hit shipment through an unlicensed bulk handling or container facility, the licensee must notify the Regional Director at the appropriate region CGC office in writing on a Notice of Direct Hit form at least 48 hours before the grain is expected to arrive at the port.
End-use certificate form 1. Canada Grain Regulations, Schedule XV., section 71
71. (1) An end-use certificate referred to in section 87.1 of the Act shall be in End-use certificate of Schedule XV.
(2) If grain is consigned directly to a milling, manufacturing, brewing, distilling or other processing facility in accordance with an end-use certificate, the operator of the facility is authorized to receive the grain.
(3) The importer referred to in an end-use certificate shall provide the Commission, within 10 days after the delivery of grain to the consignee referred to in the end-use certificate, with a copy of the bill of lading on which an authorized representative of the consignee has acknowledged receipt of the grain and the date of receipt at the consignee's processing facility.
(4) The consignee referred to in an end-use certificate shall provide the Commission, every three months after receipt of the grain referred to in the end-use certificate, with a report in the form set out in End-use certification consignee quarterly report of Schedule XV, until the grain has been fully consumed at the processing facility of the consignee.

71. (1) An end-use certificate referred to in section 87.1 of the Act shall be in end-use certificate of Schedule XV.

33. (1) Every cash purchase ticket or primary elevator receipt issued by the operator of a licensed primary elevator shall be in the appropriate form set out in Schedule V.
Interim transfer elevator receipt form 11. Canada Grain Regulations, Schedule V., subsection 58(1)
58. (1) A terminal elevator receipt, transfer elevator receipt or interim transfer elevator receipt shall be in Form 9, Form10 or Form 11, respectively, of Schedule V.
Monthly grain dealer report AGR 1772 (94/08) - This report should reflect physical handlings of grain as conducted by grain dealers.
Primary elevator annual report on non-board feed grains S2 (00/10) - The information on this document is required by the Canadian Grain Commission for the purpose of primary elevator annual reporting of net handlings of non-board feed grains.

33. (1) Every cash purchase ticket or primary elevator receipt issued by the operator of a licensed primary elevator shall be in the appropriate form set out in Schedule V.

Producer request for inspection services CGC IS-PRO1 (08/00) - This form is for producers who submit grain samples of their own production to any service center in the Prairie Region for grade certification. Grades and analyses are based only on the sample submitted.
Request for grain inspection certification IS-12-7-P (00/07) - This form is used to request that the CGC arbitrate when there is a disagreement between a grain producer and a primary elevator operator concerning the grade, dockage, moisture or protein assessment assigned to the producer's grain delivery at a primary elevator in western Canada.
