Enhanced Surveillance for Chronic Disease Program - Surveillance Grants and Contributions Funding

The Enhanced Surveillance for Chronic Disease Program supports the collection of reliable data on trends in chronic diseases and their risk factors. These data are the basis for planning public health programs and for assessing the impact of prevention on health outcomes.

The program objective is to build a comprehensive national chronic disease surveillance system that improves the federal capacity to monitor chronic disease rates and report to Canadians on progress in reducing these diseases, their risk factors and impact on health, and emerging areas that require public health attention. Tracking chronic disease rates and their risk factors is the basis for tailoring prevention programs to specific populations and measuring the success of these programs. Investments in the development and adoption of standard surveillance methods that reduce duplication and provide comparable data are key to the program's success.
