1. It is the responsibility of the employee to ensure the form is completed properly and completely
   
   
2. Once the form has been filled, print a copy to be signed by Human Resources Branch and dated. You may also wish to print another copy for your records.
   
   
3. Once these forms are completed, they are considered to be protected documents.
