Guidelines on the management of breast cancer, first published in 1998, are being revised (page 212 and page 213) and new ones are being released (page 191). Also in this issue, Ruhee Chaudry and colleagues (page 183) and Roberto Grilli (page 210) discuss how treatment setting might affect breast cancer survival.
