Step–by–Step Guide to Form 1: Contact Information

Section 1: Basic information

Number of Buildings: The number of buildings that you mark here should correspond to the information provided on Form 2, which relates to the location of human pathogens or toxins.

Legal Name of Entity (individual or organization responsible for activities involving human pathogens or toxins): In many cases, the legal name will be a corporate name. Please do not use initials or acronyms. If you are registering as an individual, your legal name is your given and family name unless you have legally changed them.

Responsible Individual: You may register as an individual. For organizations, the Responsible Individual should be authorized by the organization to represent it or an individual empowered by the organization to appoint a contact person for the purposes of the HPTA. Thus, in the case of a university, hospital, or another form of organization, the Responsible Individual might be the Head of Research, the President or a Vice-President of Research. It is possible that the Responsible Individual will also be the Contact Person described in Section 2 of the form.

Physical address of entity: In most cases, this will be a street address and should not be a post office box. This should be the same physical address that is tied to the legal name of the entity registrant, which might be the headquarters or administration building for example.

Type of Entity: From the list of the five descriptors provided, please choose up to two that best describe your entity, marking the most relevant with a “1” and the least relevant with a “2”. If your activities with human pathogens and toxins are limited to packaging, storage, and shipping, then you would check “Distributor”. “Government” includes federal, provincial, or municipal departments or agencies.

Signature by Responsible Individual: The responsible individual must sign this form. By doing so, he or she indicates that the individuals listed as the contact persons are qualified as per paragraph 70 (1) (c) of the HPTA, and that all of the information listed in the registration Forms 1 and 2 is true and complete to the best of his or her knowledge.

Section 2: Contact person

Contact Person: A point of contact must be provided with each registration. Ideally, this individual should be knowledgeable about the activities being conducted in your entity. According to the HPTA, the contact shall have “appropriate safety training in the area of human pathogens and toxins or relevant work experience.” A biological safety officer is an ideal contact but another person with a different designation and equivalent knowledge and/or experience is acceptable.

Contact’s Mailing Address: Please check the “Same as Physical Address” box if that is the case. If your institution has internal mailboxes or locators, please include these details in the mailing address.

Section 3: Alternate Contact Person

Alternate Contact: While not required, this should be an individual who is knowledgeable about the activities being conducted in your entity, capable of acting on behalf of, or as an alternate to, the main contact.

Alternate Contact’s Mailing Address: Only fill out if it is different in any way from that of the main contact.
